A transcript request must be made by the student either by letter, transcript form, fax or in person. Official transcripts (raised college seal) are $5.00. Unofficial copies are free.
Letters and fax requests must include the following information:
- full name (and all names under which you attended ACC)
- Social Security number
- date of birth
- address where the transcipts should be sent
Requests must be signed and please include a day time telephone number.
Checks and money orders should be made payable to ACC.
Charge card information should include account number and expiration date.
