2009-2010 Student Accident
and OPTIONAL
Accident and Sickness Insurance Program

Eligibility:

All full-time students and part-time students enrolled with 7 or more credit hours, and new Summer Session students, are automatically enrolled and covered by the Basic Accident Benefits described under Plan A. Only full-time students and their eligible dependents may elect to enroll in Plan B.

Plan A:

Plan A Basic Accident coverage applies to eligible enrolled students while attending or participating in any activity sponsored by the College, including work-study programs, traveling directly to or from classes or College-sponsored activities, and athletic activities.

Plan B:

Plan B provides coverage 24 hours a day during the term insured. This optional coverage includes Supplemental benefits for Accidents and Sickness.

Enrollment in Plan B:

Students may enroll themselves and their eligible dependents in Plan B by completing the enrollment form. The enrollment form may be obtained from the web-site at www.maksin.com/acc.shtml, may be downloaded and mailed to the Servicing Broker (see brochure on page 2 for address), or a brochure may be picked up at the Center for Advisement and Testing located in Warren Hall.

Additional Optional Benefits to Plan A participants:

• EYE CARE INTERNATIONAL Vision Program (automatically included in Plan B) ($12.00 fee)
