Requesting a Transcript
A transcript request must be made by the student either by letter, transcript request form, fax or in person. Official transcripts (raised college seal) are $5.00. Unofficial copies are free.
Letters and fax requests must include the following information:
- Full name (and all names under which you attended SUNY Adirondack)
- Social Security number
- Date of birth
- Address where the transcripts should be sent
Requests must be signed and please include a day time telephone number.
Checks and money orders should be made payable to SUNY Adirondack.
Charge card payments can be made through NELNET.
